This form is solely for requesting information about registering as a donor, either for yourself or someone else. Please use this if you wish to be sent an information pack, including donor and next-of-kin consent forms. If, after receiving the pack, you decide do not wish to sign up, you need do nothing further. You will not be registered as a donor until and unless we receive your completed and signed consent forms.
